Arizona Long Term Care is Medicaid that provide support for our Seniors. The 90-day or more process takes several steps and follow-up to complete the application for possible approval. Let our Licensed Social Worker hel you navigate the application process and provide Social Work Support for additional resources.
Licensed Hospice experienced Social Worker to provide grief support. Also, provide resources with end of life planning, navigating end of life choices, and support through the process and planning.

Assessment of overall BioPsychoSocial Assessment
Application, Connect with financial Worker, receives analysis of documents needed, walk through the process and ALTCS Accompany MPOA or Client for the interview.
Assistance to obtain all necessary medical Records, provide a personalized ADL assessment write up for the ALTCS Medical Accessor, Accompany MPOA or Client for the interview
Social Worker with have Continuous Follow-up with the ALTCS Eligibility Worker. Social Worker will continue to provide support until final determination.
Grief is personal. Planning can be difficult before and at end of life.
Body Donations, Funeral homes, Cremation.
Phone Calls done by Social Worker, and making Connections to mortuaries or other end of life systems. Accompany family to funeral home electronically or in-person.
Social Worker can provide additional support and resources.
Appointed by the State as an official officer of integrity. Licensed Social Worker providing witness of signature of legal documents. Medical Documents preferred. May do other documents.
Will Notarize by appointment. Mileage fee varies.
